The knee does not go into full extension during the latter part of the support phase but remains in 15–20° of flexion (Fig. 1a). Knee flexion during the early part of the support phase of running is a natural mechanism that cushions some of the force of impact. Although the peak impact forces with the ground increase with increasing running velocity (discussed below), a clear relationship between knee joint flexion and running velocity has yet to be established. Indeed, maximum knee flexion during the support phase may only increase a few degrees with increased running speed.
This explains why running in shoes usually results in more pronation than running barefoot. Manipulating the length of the moment arm by designing a flare on the medial and lateral edges of the midsole (Fig. 5b) can influence the amount of rearfoot eversion. A 15° flare on the lateral side of the midsole increases the width at the bottom of a 24-mm thick midsole in the rearfoot by 6 mm. Increasing the width in this area where the initial contact with the running surface is made increases the eversion moment arm.
In as many as 50% of elite distance runners, the arterial oxygen saturation may fall during sea level exercise at running intensities close to VO2max. The oxygen content of the blood may fall in exercise at intensities close to maximum because of a mismatch between perfusion of the vascular bed and local diffusion capacity. When the cardiac output is very high, pulmonary capillary transit time may be too short to allow complete equilibration to occur. The fact that desaturation occurs in athletes rather than in sedentary individuals may be a consequence of the small changes in lung function that occur with most types of training.
